Families at a crossroads

Most families come to us at a moment of transition — a hospital discharge, a fall, a diagnosis, or the quiet realization that things can't stay as they are.

01

A parent just got a diagnosis

Dementia, Parkinson's, or another condition that changes what care looks like. You need to understand what's ahead — and what options actually fit.

02

Hospital discharge in 48 hours

The social worker handed you a list of facilities. You have no idea how to evaluate them, and the clock is running.

03

Home care isn't working anymore

You've tried to make it work at home, but the care needs have outgrown what's possible. You need an honest assessment of what comes next.

04

You're not sure a move is needed

Maybe a community is the right answer. Maybe better home care coordination is. We'll help you figure out which — without pressure toward either.

What makes this different

Most placement services are paid to fill beds. We're paid to give you the right answer — even if that answer is to wait.

01 —

We start with the clinical picture

Before we mention a single community, we understand the diagnosis, the care needs, and what the next 12 months might look like.

Most families are handed a list of communities without anyone first asking the right medical questions. We do the assessment first.

02 —

We're independent

We don't work for any community or home care agency. Our recommendation is based on fit.

We disclose our compensation model to every family upfront. If the right answer is a community we don't have a relationship with, we say so.

03 —

We stay through the transition

Placement is not the finish line. We accompany families through tours, contract review, move-in, and 90-day follow-through.

Most advisors disappear after the move-in paperwork is signed. We check in at 30, 60, and 90 days — because that's when problems surface.

04 —

We speak the language

A healthcare administration background means we understand discharge timelines, clinical complexity, and how care systems actually work.

We can communicate directly with hospital discharge planners, case managers, and physicians — and translate that into plain language for families.

Healthcare system expertise — for every care transition

Coastal Care Advisors was founded by a healthcare administration professional who spent a career watching families get lost in a system that wasn't designed to help them.

How we are funded

In certain instances, we may receive a referral fee from the community or home care provider a family ultimately selects — making our service free to families. We disclose this to everyone upfront, and we never steer toward a higher-fee option.

Three ways we work with families

Every engagement starts with a free consultation. From there, we match the service to what your situation actually requires.
